 This laser vision benefit is available to all EyeMed Program members and their eligible dependents. Follow these simple steps to obtain your discount:
- Please call toll-free 877-5LASER6 (877-552-7376) to find a participating laser vision correction provider.
- Obtain an "EyeMed Lasik Discount Certificate".
You may choose to receive a certificate via email. Simply provide our representative with an email address and you will automatically receive your certificate via email within 24 hours.
After obtaining the surgeon(s) name and discount certificate, follow these steps:
- Schedule an appointment with an "in-network" provider.
Call the in-network provider directly and identify yourself as an EyeMed Program member. Please bring your "EyeMed Lasik Discount Certificate" to the surgeon's office.
- Schedule a treatment date.
If you and your provider determine that laser vision correction is right for you, schedule an appointment with the provider.
- Activate your discount.
Upon scheduling your treatment date, call 877-5LASER6 (877-552-7376) to activate your discount and make a $100/eye deposit.
- Payment.
The in-network provider will simply deduct your discount AND deposit from the price. Financial arrangements, if available, may be discussed directly with the surgeon's office. There are no claim forms to file.
- Post-Operative Visits.
Please follow all post-op instructions and attend all suggested follow-up visits.
You can locate a provider in your area by calling the U.S. Laser Network seven days a week, toll-free at 877-5LASER6 (877-552-7376). If you would like to receive "member instructions" and a "Lasik discount certificate", please provide the representative with your email address.
