How To Enroll
Click the "Enroll" link at the top or bottom of this site.

Provide the following required information:
- Company Name (if applicable)
- Your Name
- Address
- Social Security Number
- Phone Number
- Email Address

Include Your Payment Information. Acceptable forms of payment are:
- Visa, Mastercard, American Express Credit Cards
- Debit Cards
- Check
- Money Order
All ID cards are mailed directly to the home address provided with enrollment and will arrive approximately 3 weeks from the date enrollment and payment information has been received.
If this is a group enrollment EyeMed will not mail ID card to employer for distribution.
Renewals
Members renewing their EyeMed Access Plan D will not automatically receive a new member card. You should retain your current card for identification. Members may at any time use the automated phone system to request a new or replacement card when needed. One vision program identification card covers the entire family.
Important: An ID card is not required to access member savings as long as the member advises the participating provider that they are an EyeMed member and provide the provider the ID# (Social Security Number of registered member). The provider can verify eligibility by phone. The provider must be a participating provider in order for the discounts to be allowed and your membership must be active. Eligibility must be verified at the time of purchase. Refunds for purchases made without plan verification are not eligible for repricing. Member is responsible for verfying provider is actively participating in the network prior to purchasing eyewear.
All enrollments are good for 12 months and expire on the last day of the 12th month.

How to Access Your Discount
- Locate the EyeMed Vision provider most convenient to you. For the most up-to-date listing of EyeMed providers in your area visit the Provider Locator. To locate a provider using our 24/7 automated customer service center, please call toll-free 800-221-3272.
- When scheduling an appointment, inform the office that you are a plan member and provide your name, the name of the organization or plan (EyeMed Access Plan D) and your member ID number, which is the primary member's social security number.
Once you arrive, identify yourself as an EyeMed member and present your ID card.
- Your EyeMed provider will take care of the rest.
This plan can not be combined with any other discounts or promotional offers.
