Should you ever need a replacement ID card during your active membership, all you will need to do is call our toll free customer service number provided with your benefits material and you can order a new ID card 24/7 by using our automated voice recognition help line and following the simple instructions. It is important that you advise us of any address changes, etc. so we can keep your records up to date.
Important - Two ID cards will be included with your membership. All cards are printed with the primary subscriber's name only; eligible dependents can use one of the ID cards for identification purposes. Carry the card with you at all times and protect it as you would any important piece of identification. Please verify the provider accepts your plan when scheduling your appointment.
