What is the benefit of the Select Dental Advantage program?

The program was designed to make dental care affordable for uninsured/under insured individuals, families and groups by providing savings of 10% to 60%. In addition, there are no benefit maximums, no claim forms to submit and no waiting as this is not an insurance program.

What types of dental services are included?

Your family is eligible for savings on cleanings, x-rays, checkups, root canals, crowns, fillings, bridges, dentures, braces and more.

What about specialists?

You will have access to specialists in the field of orthodontics, prosthodontics, endodontics, periodontics and more. This also includes treatment of gum disease, bridges and oral surgery.

Can I change dentist or specialist?

Yes. You can change your dentist or specialist anytime, without notice or cost.

Will I pay more if I have pre-existing conditions?

No. Pre-existing conditions are treated at the same discounted fees.

Is this dental insurance?

No. It is simply a dental discount plan. With this plan there are no claim forms, no deductibles, no exclusions and no pre-authorizations. You can use immediately.

Before I join, can I check to see if there is a dentist that participates near me?

Yes. Before you signup, locate a participating dentist by using our provider locator service. Once you have become a member you will also be provided a toll-free provider referral number. Allow 2 weeks to process your enrollment request. You must present your ID card to the participating dentist prior to receiving treatment. In the event you require immediate treatment and you have not received your ID card, please have your dentist call us and we can verify your eligibility so that you may obtain treatment.

My dentist does not currently participate in this program; can they be added to the network?

Yes. We are always pleased to consider new dentists that would like to join the network. We cannot guarantee that every dentist you refer will qualify. There are no fees or sign up costs for the dentist to join. You may refer your dentist to us or your dentist can submit their request to join the network via this website or they can call our member services line and provide us their information over the phone. Every dentist goes through a complete credentialing process that can take up to 4 weeks depending on how quickly the dental office can provide the required information.

When is my membership effective?

Upon receiving your ID card, you are eligible to go to any participating Select Dental Advantage provider and start saving!

What's the minimum and maximum age to join the program?

There are no age limitations.

How do I start using the Select Dental Advantage Plan?

Upon receiving your ID card, you and your dependents may begin using this program immediately.

1. Locate a participating provider by using our online searchable data base or call us at 888-636-2378.
2. Call the provider and schedule an appointment.
3. The provider will call us at 888-636-2378 to verify your eligibility prior to your visit or you can call us and we will call the dentist in advance to verify your eligibility.
4. Present your ID card during each visit. All family members may use this same ID card with the primary members name as this program includes your spouse and dependents at no additional cost.

Who's covered under this plan?

All members of the immediate household including children as indicated by enrollment status filing of application (Covered Parties).

If I have a family membership, does everyone have to go to the same provider?

No, each family member may see a different participating provider and/or change participating providers as often as they wish.
